HUD-NPMA-99-B is a Pre-Treat form required by HUD for any treatments for new construction. (Updated Form – Exp: 09/30/2022)

There are two forms required (1) # 2580 NPMA 99A Builder Guarantee (completed by builder – see product 2580 NPMA- 99A) and (2) #2581 HUD-NPMA-99-B Soil Treatment Record (completed by pest control company).

Your company information can be imprinted in black ink for a small additional fee. Minimum order for imprinted forms 100.
