This Safety Checklist will take just a few minutes to to complete and provides a way to systematically check all equipment for any discrepancies before each use. In the long run, it will help save down time, money, injuries and legal hassles.

Size 5.5 x 8.5 available in 2 part (white and canary) or 3 part (white, canary and pink) carbonless forms.

Sold in Loose set or Booked 50 per book with wrap around covers. Inside cover of the Booked forms has multiyear calendar.

Forms are printed in your choice of red, green, blue, black or burgundy. Your company’s information can be imprinted where indicated, at no additional charge, (for straight type).

Company logos can be added. There is a 1 time fee for this type of artwork. $25 for first time use and $10 each new item, for repeat customers.

Once your artwork has been added to an item there will be no additional charge for Exact Repeat Orders. A proof will be sent for your approval before any printing will take place via email, fax or US postal service.

If special wording is needed to better accommodate your company’s needs, most forms can be altered for a small additional typesetting fee. (Price would depend on amount of changes made, minimum charge $10.)
