On Part B of the new Official Alabama Waiver, it states – “Note: Form consists of Part A and Part B. Both pages must be completed”

Official Alabama Waiver, Part B, (this form) offers Conditions governing the use of the waiver form, additional explanations, and graph area. Product 7164 is Part A of the newly required Official Alabama Waiver Form and this form is Part B. Part A and B are sold separately.
